DANCE ATTIRE
Pre-Combination Girls: Any color of leotard and tights with pink ballet and black tap shoes. Boys: Any color of shorts and T-shirt with black ballet and tap shoes.
Combination Classes and Ballet & Pointe Classes Girls: Black leotard, pink tights and pink ballet shoes. No skirts or leg warmers. Long hair must be neatly secured. Short hair needs head band. Boys: White t-shirt, black tights or black shorts. White socks and white ballet shoes.
All Other Classes Any color of leotard, tights, jazz pants or bike shorts. Black tap and black jazz shoes. Hair must be pulled away from face, such as in a ponytail.

REGISTRATION FEE
A $35 nonrefundable fee is required with the registration form and 1st month’s tuition to reserve class space. Registration is not complete until these fees are paid.
  • There will be a $7 discount on the second member in each immediate family.
  • Lessons are payable monthly in advance only.
  • A late fee of $10 will be charged if not paid before the 7th of the month.
  • For your convenience, monthly tuition may be charged automatically to a credit card. (See the office)
  • Tuition is not refundable.
  • Tuition remains the same regardless of absences or holidays. Since some months are longer than others, the extra classes received equal those missed for holidays.
  • In case of illness or emergency, classes may be “made up” if approved by the teacher.
  • There will be a $25 service fee for all returned checks.
  • Credit Card Returns will be charged a 10% fee.
  • Statements will not be sent unless account becomes past due.*
    *All delinquent accounts will be processed through Small Claims Court and all fees will be added.*


INSURANCE
We do not carry medical insurance for our students. It is required that all dance students be covered by their own family policies and if injury occurs it is understood that the student’s own policy is the only source of reimbursement.

SPRING CONCERT
MDA has an annual recital at the end of the school year. For each student participating, there is a nonrefundable $50 recital fee and an approximate $40 to $75 costume fee per class. Balances on costumes must be paid before ordering day. A $10 late charge will be added per costume after ordering date to cover postage and handling costs. Students are required to attend all dress rehearsals prior to the show.
